Changes to Participating Providers' Information

Can your patients find you? Are your claims being processed correctly? Does the PPO USA Network have accurate and up-to-date information about you?

If you want to make sure the answers to your questions are "yes," you may check your information by selecting Provider Lookup. Select the type of provider for which you are searching, then select either Search by Name or Search by Location and Specialty. Enter the search criteria to locate your practice information.

If your name, address or telephone number is incorrect, or if one of your locations is not listed in our online directory, please submit the updated information to us in writing. Although your tax identification number is not listed on our website (for confidentiality purposes), you may call us to verify that we have the correct number. To change your tax identification number, please send us your updated information in writing or submit a copy of your W-9 to us.

To notify us in writing of any additions or changes, you may email us, or send your correspondence to the address or fax number listed under Provider Relations Staff. Please supply us with as much detail as possible.

Providing us with your current information will enable us to have even more opportunities to promote you and direct members to you through our website referrals, telephone referrals and printed directories. It is also important for us to have accurate information about you so we can make sure claims are being processed correctly.
